Position Overview
The Head of Case Management Services is accountable for the strategic planning, oversight, and administration of the Case Management Program at Salem Hospital, Inc. This role encompasses the development and management of a skilled team, including Registered Nurses (RNs), Social Workers (SWs), and non-clinical personnel. The Director will evaluate and enhance case management competencies and outcomes, oversee policy formulation, and ensure compliance with essential model components that influence patient outcomes.

Key Responsibilities
- Collaborate with multidisciplinary teams to guarantee high-quality, timely, and cost-effective patient care.
- Develop and mentor staff to foster professional growth and ensure optimal performance.
- Utilize strong analytical skills to strategically plan, build consensus, and resolve issues effectively.
- Communicate complex information clearly and professionally to various stakeholders, including patients, families, and healthcare professionals.
- Manage and analyze clinical and financial outcomes to drive improvements in service delivery.

Qualifications
Required:
- Proven ability to interact with diverse populations and adapt communication styles accordingly.
- Minimum of 7 years of acute clinical experience.
- RN with a Master's Degree and a current license as a Registered Nurse in Massachusetts.
- Strong proficiency in data analytics and computer applications, including hospital-specific software and Microsoft Office.

Preferred:
- At least 5 years of progressive management experience with a focus on achieving measurable outcomes.
- Demonstrated leadership in change management and case management practices.
