Job Summary

The LPN - PTI (Per Diem) will be responsible for monitoring the health, safety, and well-being of program participants in our respite home. This role requires the ability to provide direct nursing care, manage medical records, and communicate effectively with families, clinicians, and other stakeholders.

Key Responsibilities

  • Monitor the health of program participants and provide technical assistance regarding nursing care and medical treatment as needed.
  • Manage and document medical information, referrals, reports, medication administration, and data collection.
  • Orient and train staff on understanding participants' medical needs and following prescribed programs and plans.
  • Provide leadership and support during medical crises.
  • Ensure professional communication with families, DSS, clinicians, and others for assigned participants.
  • Process all human rights issues, incident, and accident reports in a timely manner according to agency policy/procedure.

Requirements

  • Licensed Practical Nurse registered with the MA Board of Registration in Nursing Required.
  • At least 1 year of experience working with the elderly or persons with developmental disabilities.
  • Strong interpersonal, written, and verbal communication skills required.
  • Valid driver's license and reliable transportation.
