Job OverviewPosition: Full-Time Youth Housing Support Specialist
General Overview: The Youth Housing Support Specialist will engage with young adults aged 18-24 within the Thrive Housing Program. Responsibilities will encompass collaboration with CoC and/or YHDP case managers to facilitate client service plans.
Key Responsibilities - Housing Transition Planning & Coordination
  • Participate in post-housing program planning for youth nearing the conclusion of their housing program (ensuring proper notice has been provided to the apartment or lease renewal is in place).
  • Assist in housing search and placement, coordinating move-in processes with case managers to ensure a seamless transition.
  • Conduct exit surveys with youth, uploading data to HMIS, documenting final move-out or lease renewal plans, and explaining aftercare processes to ensure youth are aware of available resources.
  • Communicate with case managers regarding any additional concerns as the housing program exit approaches (e.g., youth requiring group-home placement or lacking employment plans).
  • Perform check-ins as needed or requested by case managers with youth, either in person or via phone.
  • Provide aftercare services to clients through various communication methods, offering necessary resources.
  • Maintain and update all relevant HMIS data and documentation, including a housing-exit summary case note for each youth.
  • Submit referral forms for benefits or educational/employment assistance as required.
  • Assist clients in obtaining identification documents.
  • Research and identify resources needed for clients.
  • Utilize on-campus workspace when not engaged in youth meetings.
  • Ensure timely completion of required documentation and accurate entry of information into the appropriate web-based systems and HMIS.
  • Adhere to all policies and procedures as outlined in the Employee Handbook and departmental manuals.

Additional Responsibilities:
  • Must have daily access to a vehicle without prior notice.
  • Maintain current vehicle registration and liability insurance.
  • Be available to transport clients to various appointments (mileage will be reimbursed).
  • Pass a criminal background check.
  • Have access to a cell phone.
  • Occasionally participate in activities beyond regular responsibilities.
Education Requirements: Bachelor's Degree in Social Work, Psychology, or a related field.
Experience Requirements:
  • Minimum of two years of experience in case management services within community programs serving youth, at-risk young adults, and/or homeless individuals.
  • Familiarity with community resources.
  • Understanding of trauma-informed care principles.
Preferred Qualifications:
  • CPR & BLS Certification.
  • Knowledge of HMIS.
  • Fluency in English and Spanish is preferred.
Required Skills:
  • Strong organizational skills.
  • Proficiency in computer applications, particularly Microsoft Office.
  • Excellent communication skills.
  • Problem-solving and critical thinking abilities.
  • Flexibility regarding employment responsibilities, duties, and scheduled hours.
  • Daily access to a motor vehicle for client travel (mileage will be reimbursed).
