Summary

The YHDP TH-RRH Case Manager is a seasoned social service professional tasked with delivering field-based, direct client support to a designated group of individuals enrolled in the Transitional Housing – Rapid Re-housing Joint Program. This initiative caters to youth and young adults aged 18-24 who are experiencing homelessness, allowing them to choose between Transitional Housing or Rapid Re-housing based on their unique needs and preferences, utilizing the Housing First approach.

The primary focus of case management will be to stabilize clients upon entry into the program, ensuring their needs are identified and addressed appropriately. Responsibilities include securing and maintaining permanent housing, supporting landlord-tenant relationships, connecting clients to educational and employment resources, advocating for youth, linking clients to mainstream services, and assisting clients in successfully exiting the program to achieve self-sufficiency.

Key Responsibilities

  • Deliver mobile case management services covering areas such as independent living skills, housing stabilization, financial management, community integration, employment connections, and establishing benefits.
  • Provide strengths-based, client-centered case management and service coordination aimed at helping clients achieve and sustain stable employment and housing.
  • Collaborate with clients to create individualized service plans that address both short-term and long-term objectives.
  • Develop a projected budget plan to help clients understand affordable housing options post-assistance.
  • Enhance clients' abilities to independently solve problems, utilize effective coping strategies, and manage their own care.
  • Implement evidence-based practices in service delivery, including intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, Trauma-Informed Care, and Housing First methodologies.
  • Work alongside clients to formulate and update housing stability plans based on comprehensive assessments.
  • Utilize various interventions to help clients overcome barriers to housing stability, including increasing income and benefits.
  • Assist clients with housing searches and placements, including accompanying them to view potential apartments and connecting them to community resources.
  • Advocate for clients with landlords to resolve any issues that may impede securing and maintaining suitable housing.
  • Mediate disputes between clients and landlords that could jeopardize housing stability.
  • Assess and connect clients to relevant resources such as support groups, training sessions, workshops, and classes to enhance life skills, including financial literacy, education, job preparedness, and health and wellness.
  • Assist clients with budgeting and provide referrals for financial counseling as necessary.
  • Engage in case conferencing, staff meetings, and training sessions.
  • Maintain regular face-to-face contact with clients to review case plans and ensure adherence to program policies and procedures.

Documentation Responsibilities

  • Ensure accurate and timely documentation of case management services in compliance with program contracts and organizational policies, including adherence to HIPAA regulations.
  • Complete intake assessments and service plans in collaboration with clients, following documentation standards.

Qualifications

  • Experience in maintaining confidentiality in accordance with HIPAA standards.
  • A minimum of one year of experience in homeless services, mental health, substance use, or working with vulnerable populations providing intensive case management.
  • Knowledge of housing and case management techniques, including critical time intervention, harm reduction strategies, and motivational interviewing.
  • Ability to understand family dynamics in crisis and de-escalate conflicts while working with diverse populations.
  • Experience with young adults, LGBTQ, and diverse groups is preferred.
  • Flexibility to work evenings and weekends as required.
  • Experience with HMIS is a plus.
  • Possess a high level of empathy and understanding for individuals with complex needs.
  • Ability to work independently and collaboratively within a team environment.
  • Strong written and verbal communication skills.
  • Self-motivated with the ability to manage multiple tasks in a high-pressure environment.
  • Driving is an essential function of this position, requiring a valid driver's license and reliable transportation.

Education and Experience

A Bachelor's degree in social work or a related field, along with a minimum of one year of case management experience.

Language Skills

Bilingual skills (English/Spanish) are preferred.

Mathematical Skills

Ability to perform basic calculations.

Certificates, Licenses, Registrations

A Class C driver's license is required. Licensure as a social worker or equivalent is desirable.

Work Environment

Employees may interact with individuals and families in crisis, requiring readiness to respond to various situations, including those involving substance use and personal hygiene challenges. The work environment may be stressful at times, necessitating flexibility in work hours.

Physical Demands

Employees must be able to perform essential functions, including standing, sitting, walking, and using manual dexterity. Occasionally, lifting up to 20 lbs may be required.
