SUMMARY
The YHDP TH-RRH Case Manager is a skilled social service professional dedicated to delivering direct client support in the field to a designated group of individuals participating in the Transitional Housing – Rapid Re-housing Program. This initiative serves youth and young adults aged 18-24 who are facing homelessness, offering them the opportunity to engage in either Transitional Housing or Rapid Re-housing tailored to their specific needs and preferences, following the Housing First approach.
Case management responsibilities will prioritize the stabilization of clients upon their entry into the program, ensuring their needs are recognized and addressed appropriately. This includes securing and maintaining permanent housing, fostering positive landlord-tenant relationships, connecting clients with educational and employment opportunities, advocating for youth, linking clients to essential services, and supporting them in successfully transitioning out of the program towards self-sufficiency.
KEY RESPONSIBILITIES
- Deliver mobile case management services focusing on independent living skills, housing stabilization, financial management, community integration, employment connections, and access to essential services for mental health and substance use.
- Implement strengths-based, client-centered case management and service coordination aimed at helping clients achieve and sustain stable employment and housing.
- Collaborate with clients to create personalized service plans that outline both short-term and long-term objectives.
- Assist clients in developing budget plans that reflect affordable housing options post-assistance.
- Enhance clients' abilities to independently solve problems, utilize effective coping strategies, and manage their own care.
- Employ evidence-based practices in service delivery, including intensive case management, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
- Work in partnership with clients to formulate and revise housing stability plans based on ongoing assessments.
- Utilize various interventions to help clients overcome obstacles to housing stability, including increasing income and accessing benefits.
- Support clients in their housing search and placement, including accompanying them to view potential residences and connecting them with local resources.
- Advocate for clients with landlords to address challenges in securing and maintaining appropriate housing.
- Mediate disputes that may jeopardize housing stability.
- Connect clients to relevant resources such as support groups, training sessions, and workshops to enhance life skills.
- Assist clients with budgeting and provide referrals for financial counseling as necessary.
- Engage in case conferencing, staff meetings, and professional development training.
- Maintain regular face-to-face contact with clients to review case plans and ensure adherence to program guidelines.
DOCUMENTATION
- Ensure accurate and timely documentation of case management services in accordance with program standards and organizational policies.
- Complete intake assessments and service plans collaboratively with clients, adhering to documentation protocols.
QUALIFICATIONS
- Experience in homeless services, mental health, or working with vulnerable populations is essential.
- Knowledge of housing and case management strategies, including crisis intervention and motivational interviewing.
- Ability to work effectively with diverse populations and manage conflict situations.
- Flexibility to work evenings and weekends as required.
- Strong communication skills, both written and verbal.
- Valid driver's license and reliable transportation are necessary for this role.
EDUCATION and EXPERIENCE
A Bachelor's degree in social work or a related field, along with a minimum of one year of case management experience, is required.
WORK ENVIRONMENT
The role may involve interaction with individuals and families in crisis, necessitating a readiness to respond to various situations, including those that may be challenging or stressful.
