As a vital member of our organization, the Case Manager is responsible for delivering comprehensive support to clients, ensuring their needs are met effectively.
Key Responsibilities:
1. Conduct thorough evaluations to assess the needs of clients and their families.
2. Collaborate in the formulation of personalized service plans that align with client assessments.
3. Facilitate the provision of essential services identified during assessments.
4. Aid clients in navigating access to necessary services through referrals and connections.
5. Track and evaluate client progress by coordinating appointments, following up on services, and reassessing needs periodically.
6. Advocate for clients by engaging in activities such as writing letters to expedite benefits and empowering clients and their families to advocate for themselves.
7. Maintain detailed case records that document interactions, required services, progress reports, and other relevant information in compliance with organizational and regulatory standards.
8. Provide case consultation by collaborating with service providers to assess client status and progress.
9. Communicate effectively with supervisors regarding case management issues, resource allocation, and service collaboration.
10. Deliver psychosocial education to clients and families to enhance their understanding of various conditions and situations.
11. Offer crisis intervention services, including emergency referrals and coordination of urgent support services.
12. May take on the role of Primary Case Coordinator, with responsibilities varying based on the complexity of cases and the individual's expertise.
Qualifications:
• A Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field recognized by Medicaid or relevant funding sources.
• Completion of a state-mandated case management/service coordination course within six months of employment.
Experience:
ADULT DIVISION: A minimum of one (1) year of full-time experience post-Bachelor's degree working directly with adults or the ACT population in a human services environment; relevant Master's degree holders are exempt from this requirement.
