The selected candidate will play a crucial role in enhancing patient management quality, ensuring satisfaction, and promoting cost efficiency. Responsibilities include overseeing the complete patient care cycle, which encompasses assessment, planning, implementation, monitoring, and evaluation of necessary actions to meet clients' health and human service needs. Certification in case management is advantageous.

Key Responsibilities:

  • Deliver care that is safe, timely, effective, efficient, equitable, and centered around the client
  • Manage case assignments, develop service plans, assess case progress, and determine case closure
  • Assist clients in achieving wellness and independence
  • Facilitate various aspects of care, including coordination and information sharing
  • Act as an advocate for patients, helping them make informed decisions regarding their treatment options
  • Build effective relationships and collaborate with the medical team throughout the case management process
  • Engage with patients to monitor their progress and ensure satisfaction
  • Document case information accurately, complete necessary forms, and generate statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Comply with professional standards as outlined by established protocols, rules, and regulations

Qualifications:

  • Demonstrated experience in case management, nursing, or related healthcare roles
  • Strong understanding of case management principles, healthcare management, and reimbursement processes; knowledge of Medicaid is a plus
  • Excellent communication abilities
  • Outstanding organizational and time management skills
  • Proficient problem-solving skills and capability to manage multiple tasks
  • Compassionate and effective in teamwork
  • Current license, certification, or registration in the relevant field
  • Bachelor's degree in a related healthcare discipline
