Position Overview:
Reports to the Care Coordination Manager and collaborates with healthcare providers, interdisciplinary teams, and clinical staff to assist patients with chronic illnesses and complex health needs. This role adheres to established guidelines from primary care and clinical programs, ensuring effective communication, service coordination, and patient education.

Key Responsibilities:
  • Work in partnership with healthcare providers and interdisciplinary teams to support patients with chronic conditions.
  • Facilitate clear communication and coordinate services to address patient barriers.
  • Provide education and guidance on health concerns to patients.
  • Document care management activities accurately in the patient’s health record.
  • Identify patients requiring care management services based on specific health criteria.
  • Respond to referrals and assess patient needs for comprehensive care.
  • Deliver group education sessions for patients with chronic health issues.
  • Maintain compliance with OSHA and HIPAA regulations.
  • Participate in meetings and collaborate with team members to enhance patient care.
Qualifications:
  • Current RN license in the state.
  • Degree in nursing or a related field.
  • Three to four years of experience in a medical setting.
  • Care Management Certification is preferred.
Skills Required:
  • Strong organizational and communication skills.
  • Ability to manage multiple tasks effectively.
  • Proficient in electronic health records and Microsoft Office applications.
  • Experience in patient education and motivational interviewing.
