Job Summary

Sea Mar Community Health Centers is seeking a dedicated and compassionate Case Manager to join our Behavioral Health team. As a Case Manager, you will play a vital role in providing support and guidance to clients in our Housing Program, helping them achieve their goals and improve their overall well-being.

Key Responsibilities
  • Provide outreach services to prospective clients, coordinating efforts with outside agencies and working collaboratively with colleagues and community partners.
  • Offer peer counseling and support, drawing on your own experiences to validate clients' feelings and encourage them to take an active role in their recovery.
  • Serve as a mentor to clients, promoting hope and empowerment as they work towards their goals.
  • Conduct ongoing psychosocial assessments to identify clients' psychological, social, vocational, and economic needs.
  • Monitor and submit data, reports, and paperwork for the PATH program, while also assisting clients in obtaining housing.
Requirements
  • Bachelor's degree in Counseling, Social Work, Psychology, or a related field (or equivalent experience).
  • Agency Affiliated Counselor certification in the State of Washington.
  • Active Certified Peer Support Certification in the State of Washington.
  • Valid driver's license and access to a car.
  • Ability to pass a TB test and maintain current Basic Life Support (BLS) CPR certification.
