Job Summary:The Case Manager will work collaboratively with our mental health team to provide housing support interventions and referrals to community providers. This role requires strong communication and interpersonal skills, as well as the ability to work autonomously and as part of a team.
Responsibilities:- Provide outreach services to prospective clients and coordinate outreach efforts with outside agencies.
- Offer peer counseling and support to clients, drawing on common experiences to validate their experiences and provide guidance and encouragement.
- Serve as a mentor to clients to promote hope and empowerment.
- Participate in ongoing psychosocial assessment of clients' goals and needs.
- Monitor, compile, and submit data, reporting, and paperwork for the PATH program.
- Prefer a Bachelor's degree in Counseling, Social Work, Psychology, or a related field.
- Must be an Agency Affiliated Counselor in the State of Washington.
- Must hold an active Certified Peer Support Certification in the State of Washington.
- Must have access to a car, a valid driver's license, and current auto registration.
