SSVF Screening Coordinator

At Veterans Inc., we are seeking a highly motivated and skilled individual to fill the SSVF Screening Coordinator position. As a key member of our team, you will play a crucial role in assessing veterans' eligibility for enrollment in our Rapid Rehousing or Homeless prevention services and making appropriate referrals/linkages to community services for ineligible households.
Key Responsibilities:- Perform initial screening and assessments of all veterans for acceptance into Veterans Inc. programs for all New England.
- Provide a strength-based approach to homeless diversion, rapid resolution, and CES efforts.
- Maintain an active role in VA and community meetings, fostering collaborative working relationships with community partners.
- Coordinate staff in homeless Veteran outreach and engagement to identify homeless Veterans for rapid resolution interventions.
- Develop and maintain relations with local social services, public benefit agencies, faith-based and community-based organizations, VA facilities, Vet Centers, and Veterans Benefit Administration (VBA) offices.
- Establish linkages with appropriate agencies and service providers in the area/community.
- Coordinate veterans' participation in the agency programs and outside referrals.
- Participate in trainings and workshops as directed by the SSVF program Manager.
- Complete record keeping/documentation responsibilities and compile data for monthly reporting as required.
- Conduct outreach activities to market Veterans Inc. programs and engage new referral sources.
- Bachelor's degree in Human Services, Social Work, or other related field required.
- Minimum of two years' experience in Human Services, Vocational Rehabilitation, and Counseling, Social Work, or relevant area required.
- Experience working in a human services setting, veteran, or homeless population.
- Must be able to pass a CORI background records check.
