Job Summary:

As a Nurse Practitioner with CharterCARE Health Partners, you will play a vital role in delivering high-quality patient-centered care. Your expertise will enable you to provide a wide range of primary medicine services, including diagnosis, treatment, and coordination of care. You will work collaboratively with our team to ensure that our patients receive the best possible care.


Responsibilities:
  • Provide comprehensive primary care services to patients, including health maintenance and preventative care.
  • Diagnose and treat acute and chronic illnesses, as well as provide routine check-ups and health screenings.
  • Coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
  • Develop and implement individualized care plans to meet the unique needs of each patient.

Requirements:
  • Master's Degree in Nursing from an accredited program.
  • Current RI RNP License, Federal DEA License, and State of Rhode Island controlled substance license.
  • 1-2 years' experience as a nurse practitioner in a specified specialty, preferred.
  • Ability to work cooperatively with peers and other disciplines.
  • Strong organizational and priority-setting skills.

Working Conditions:

Our Nurse Practitioners work in a well-lighted and well-ventilated area, with frequent patient contact requiring adherence to universal precautions. May be required to lift heavy objects weighing 25 pounds from time to time.
