Job Summary:

ConcertoCare is seeking a Licensed Practical Nurse (LPN) to join our multidisciplinary Field Team, delivering home- and community-based care to patients with complex needs. As a key member of our team, you will have the opportunity to engage in top-of-license clinical practice, collaborate with a multidisciplinary team, and access professional development opportunities.

Responsibilities:

Partner with team members to develop and implement clinical treatment plans under the supervision of providers and registered nurses.
Provide clinical services and coordination in the home and field settings, including co-visits, telehealth visits, and independent LPN visits.
Conduct health screenings for newly enrolled patients and ongoing assessments.
Perform clinical services, such as wound care, phlebotomy, medication review, and patient status checks.
Meet with unable-to-reach patients to re-engage them in the program and provide cross-coverage for team members.

Requirements:

Licensed Practical Nurse with certification in the state of employment.
Experience providing care for patients with complex needs, including mental illness and substance use disorders.
Current CPR and BLS certification required.
Average to advanced computer skills and ability to travel as needed.
