Role Summary: We are seeking a Licensed Practical Nurse (LPN) to deliver health services and direct care to individuals with disabilities residing in a residential program. This role includes acting as an advocate and liaison with community healthcare providers, overseeing medical records, and managing medication administration.

Work Schedule: Monday - Friday, 11:00 PM - 7:00 AM (Awake) (40 hours)

Key Responsibilities:

  • Deliver specialized nursing care and daily support to individuals.
  • Document healthcare services in compliance with state regulations, nursing standards, and LPN licensing requirements. Adhere to all agency policies and procedures.
  • Operate under the guidance of the program director, registered nurse, and Director of Health Care.
  • Serve as a liaison to community agencies and treatment providers regarding medical needs and care. Assist other staff in identifying suitable community health resources.
  • Provide in-service education to individuals and/or staff on health-related topics.
  • Administer medications to assigned individuals following the Department of Public Health Medication Administration Program (MAP) guidelines.
  • Conduct quality assurance reviews of medication systems and ensure MAP compliance.
  • Collaborate with the Program Director and registered nurse, evidenced by regular meetings for medical updates.
  • Engage with Bridgewell's nursing staff through participation in monthly meetings and agency-wide healthcare planning.
  • Work with the Program Director and Registered Nurse to assess, organize, update, and maintain healthcare and medication needs, including:
    • Individual client assessments
    • Review of routine and other physical examinations
    • Review of physician-ordered treatments
    • Ensuring follow-up on treatments and coordination of medical and psychiatric care
    • Maintenance of medical records.
  • Perform other assigned duties as necessary.

Direct Support Responsibilities:

  • Assist individuals with all activities of daily living, including grooming, bathing, oral hygiene, feeding, and toileting.
  • Provide care such as monitoring vital signs and applying creams/ointments.
  • Ensure the comfort, privacy, and dignity of each individual during service delivery.
  • Monitor residents for changes in condition or behavior and report these changes to the Program Director.
  • Deliver services in accordance with DDS, CARF, and all applicable regulatory standards.
  • Support individuals with medical, clinical, and financial needs by identifying requirements and securing appropriate services.
  • Transport individuals to and from medical, recreational, and other activities as needed.
  • Assist with housekeeping duties to maintain a safe and clean living environment.
  • Maintain progress notes in compliance with DDS and all relevant regulatory standards.
  • Write annual assessments on individual goals as outlined in the Individual Service Plan (ISP) and participate in ISP meetings.
  • Help individuals access community resources and recreational opportunities.
  • Maintain core training certifications including CPR/FA, OSHA, CPI, MAP, and PABC as applicable.

Qualifications:

  • Graduation from an accredited and approved licensed practical nurse program.
  • Current Massachusetts Nursing license required.
  • Two years of general nursing experience and/or experience serving individuals with developmental disabilities in a community health setting preferred.
  • Experience working with adults with developmental disabilities and behavioral challenges is a plus.
  • Valid Driver's license with a good driving record for at least one year required.
  • Ability to drive a 12-passenger van; training will be provided.
  • Acceptable results of a CORI check according to Bridgewell policy are required.

  • Strong written and verbal communication skills.
  • Excellent organizational and problem-solving abilities.
  • Ability to build positive relationships.
  • Basic to intermediate computer proficiency is necessary.
