Position Overview

The Physical Therapist role at Hathorne Hill Rehabilitation and Healthcare Center involves delivering high-quality care in a 120-bed skilled nursing environment.

Qualifications
  • Must be a graduate from an accredited institution with a B.S., Doctorate, MA, or MS in Physical Therapy recognized by the APTA.
  • Current licensure and/or registration in the applicable state is required.

Key Responsibilities
  • Conduct thorough physical therapy assessments based on physician directives.
  • Accurately document findings using standardized evaluation forms while adhering to established guidelines.
  • Develop comprehensive treatment plans that include both long-term and short-term objectives, frequency, duration, and modalities of treatment.
  • Ensure that physician orders are secured for evaluations, treatments, re-certifications, and discharges.
  • Administer treatment to patients using various modalities, adjusting treatment as necessary while following safety protocols.
  • Maintain accurate and timely documentation of patient participation and progress through weekly progress notes.
  • Update treatment plans promptly as required.
  • Prepare discharge summaries and document any caregiver or discharge in-services as necessary.
  • Complete all required documentation forms thoroughly.
  • Document care plans and relevant information for the MDS as needed by the facility.
  • Attend facility and rehabilitation meetings as directed by the Supervisor or Regional Manager.
  • Engage in facility committee meetings as required.
  • Act as a rehabilitation representative within the interdisciplinary team.
  • Assist in patient scheduling as needed.
  • Conduct screenings as indicated.
  • Ensure ethical and accurate billing for physical therapy services provided.
  • Cover the caseload of other therapists during their absence when necessary.
  • Supervise junior staff members or students as required, co-signing notes as needed.
  • Foster professional relationships with colleagues, facility staff, patients, and their families.
  • Provide in-service education on patient care or physical therapy services to rehabilitation staff, facility personnel, hospitals, and/or the community as needed.
  • Adhere to all facility policies and procedures.
  • Maintain patient confidentiality and comply with Federal Resident Rights.
  • Report to work punctually, adhere to scheduled hours, and maintain a professional demeanor at all times.
  • Meet productivity standards.
  • Follow all HR policies regarding breaks, overtime, sick leave, and time-off notifications.
  • Contribute to a positive work environment by communicating professionally and fostering good relationships with customers, clients, colleagues, and management.
  • Perform any additional duties assigned by the department supervisor, lead therapist, or regional manager.
  • Comply with all written and verbal policies and procedures.
  • Be available for travel to nearby facilities when required.
