Key Responsibilities:

As a Lead Care Partner, you will:

  • Assist residents with daily living activities, including personal hygiene, mobility, and engagement in enriching activities.
  • Participate in the recruitment and training of new hires, ensuring they receive adequate support and guidance during their onboarding process.
  • Support the neighborhood director in staff scheduling and managing care team shifts.
  • Provide ongoing assistance and problem-solving support to the care management team throughout the shift.
  • Ensure that care requests from residents are addressed promptly and compassionately.
  • Oversee housekeeping and laundry services to maintain a clean and comfortable environment for residents.
  • Ensure accurate documentation of care activities and facilitate effective communication during shift changes.
  • Assist in maintaining dietary services during meal times, ensuring residents receive their meals as needed.
  • Communicate any changes in residents' health or behavior to the appropriate team members.
  • Contribute to the development of individualized service plans for residents.
  • Provide feedback on staff performance and identify needs for supplies and maintenance.
  • Maintain a clean and organized work environment.
  • Stay informed about emergency procedures and take a leadership role when necessary.
  • Attend meetings and training sessions for professional development.

Required Skills and Qualifications:

  • Ability to present information clearly and respond to inquiries from various stakeholders.
  • Knowledge of relevant policies and procedures.

  • Problem-solving skills and adaptability to various situations.

Additional Requirements:

Candidates must:

  • Communicate effectively with individuals of all ages and backgrounds.
  • Read and write in English and follow instructions accurately.
  • Maintain a professional appearance and adhere to the dress code.
  • Be flexible with work hours, including weekends and evenings.
  • Demonstrate good health and physical capability to perform job duties.
  • Pass a health screening for tuberculosis and a criminal background check.
  • Hold a high school diploma or equivalent.
  • Possess Nurse Aide Certification or equivalent.
  • Be at least 18 years old, unless certified as a Nurse Aide in Virginia.
  • Have a minimum of one year of experience as a Certified Nurse Assistant, with familiarity in geriatric care.

Mental and Physical Demands:

This role requires:

  • Regular communication and auditory skills.
  • Specific vision capabilities, including close vision.
  • Physical activity including standing, walking, and lifting up to 25 pounds.
  • The ability to push weights of at least 200 pounds over a distance.
  • Capability to assist in emergency evacuations if necessary.
  • Ability to maintain a positive demeanor in a dynamic work environment.
