Key Responsibilities- Resident Care and Services: Identify ongoing needs and services of residents, ensuring the delivery of high-quality resident services.
- Healthcare Services Coordination: Participate in the evaluation and documentation of residents with changes in healthcare services and status, ensuring proper follow-through and communication with residents, families, and healthcare providers.
- Wellness Visits and Monitoring: Conduct monthly wellness visits for all residents, monitoring and following up on any falls, accidents, or incidents, and documenting and reporting as required.
- Communication and Collaboration: Maintain open communication with residents' families and responsible parties to ensure medical needs are met, and collaborate with the Resident Care Director and community team to promote infection control standards.
- Medication and Treatment Administration: Regularly monitor and administer medications and treatments as ordered, ensuring accurate documentation and compliance with regulations.
- Education and Licensure: Graduate of an approved college or school of nursing, with a current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Skills and Abilities: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, federal, state, and local regulations, good assessment skills, ability to handle multiple priorities, and proficiency in computer skills, Microsoft Office, and Sunrise applications.
