Overview & Responsibilities

As a Wellness Nurse, you will provide direct nursing care to residents while overseeing the care provided by medication care specialists. Your responsibilities will include administering medications, performing treatments, obtaining specimens, and completing necessary documentation. The following duties are representative of the essential functions of this role:
  • Document resident status in accordance with policy.
  • Supervise assigned team members while delivering nursing care to residents.
  • Assist with the onboarding of new residents.
  • Support residents with Activities of Daily Living (ADL) and promote their independence.
  • Respond promptly and courteously to resident calls.
  • Identify medical and behavioral changes in residents and develop care plans accordingly.
  • Report daily to the Community Wellness Director on conditions, performance, and supply needs.
  • Administer nursing care per physician's orders and established standards.
  • Ensure comprehensive nursing services are coordinated with other disciplines.
  • Assist physicians during rounds and examinations.
  • Assess resident conditions and notify relevant parties of any changes.
  • Administer prescribed medications accurately and supervise nursing care continuity.
  • Provide leadership and supervision to team members, ensuring quality care.
  • Monitor complex health statuses and adjust care plans as necessary.
  • Orient residents and staff to emergency procedures and respond to emergencies.
  • Complete incident reports and follow up as required.
  • Ensure accuracy and completeness of resident records.
  • Monitor the environment to maintain safety and sanitation standards.
  • Embrace a culture of hospitality in all interactions.
  • Adhere to Residents' Rights policies consistently.
  • Observe community policies and procedures.
  • Accept assigned duties and provide feedback professionally.
  • Perform incidental housekeeping tasks to maintain a pleasant environment.
  • Carry out other related duties as assigned in a timely and professional manner.
Qualifications:
  • Current nursing license in the state of employment.
  • Minimum one year of nursing experience, preferably with dementia care.
  • Strong interpersonal and teamwork skills.
  • Ability to process written information and observe resident behaviors.
  • Exhibit high personal integrity and compassion for older adults.
  • Annual Flu vaccine required, with exemptions considered.
