Position Overview
We are currently seeking a Care Navigator II for our Outpatient Program. This role involves coordinating referrals and engaging families in our services, under the guidance of the Care Navigator Manager. Your contributions will play a vital role in achieving the goals outlined in treatment plans.

Key Responsibilities
- Receive and assess referrals from various sources, including the Call Center and Program Coordinators.
- Engage families promptly, ideally within one business day of referral receipt.
- Complete necessary intake paperwork and ensure timely client onboarding.
- Utilize culturally competent strategies to connect with families through various communication methods.
- Collaborate with the care team to assign families to appropriate direct care staff.
- Collect and manage data for treatment outcomes and reporting purposes.
- Ensure all referrals are pre-screened and documented accurately.

Qualifications
- A related Bachelor's degree or an Associate's degree with two years of relevant experience, or six years of direct service experience with children and families.
- Experience in an administrative role within a healthcare setting is preferred.
- Bilingual/Bicultural (Spanish/English) is required.

Skills and Abilities
- Strong communication and organizational skills.
- Ability to work independently and make sound decisions.
- Proficiency in Microsoft Office applications and data management.
- Knowledge of community resources and ability to connect families with necessary services.

Physical and Mental Requirements
- Ability to perform various physical tasks such as sitting, walking, and standing throughout the workday.
- Must be able to travel to client locations as needed.
