Position Overview: Under the supervision of the Care Navigator Manager, the Care Navigator II is responsible for managing and coordinating referrals for all outpatient programs while engaging families in our services.

Key Responsibilities:

  • Receive and review referrals from various sources, including the Call Center and Program Coordinators.
  • Engage families within one business day of referral receipt.
  • Open client cases within ten business days and complete necessary intake paperwork.
  • Utilize culturally competent strategies to connect with families through various communication methods.
  • Collaborate with the Clinical Program Manager to assign families to appropriate care staff.
  • Support data collection and sharing for treatment outcomes and reporting requirements.
  • Ensure all referrals are pre-screened and maintain accurate documentation.
  • Assist in the coordination of departmental events and provide excellent customer service.
  • Maintain confidentiality and adhere to all relevant policies and procedures.

Qualifications:

Applicants should possess a related Bachelor's degree or an Associate's degree with two years of relevant experience, or six years of direct service experience with children and families. Experience in a mental health setting is preferred, along with bilingual capabilities in Spanish and English.

Skills Required:

  • Strong communication and organizational skills.
  • Ability to work independently and make sound decisions.
  • Proficiency in Microsoft Office applications.
  • Knowledge of community resources and the ability to link families to necessary services.
