Position Overview:
PeaceHealth is in search of a dedicated Clinical Pharmacist for a Full Time, 0.88 FTE, Night role. The compensation for this position is competitive, reflecting the experience and qualifications of the candidate.

This role is essential in ensuring the effective use of medications to enhance patient outcomes and improve their quality of life.

Key Responsibilities:
The Clinical Pharmacist will engage in a variety of functions aimed at delivering compassionate pharmaceutical care, which includes:

  • Assessing medication orders for correctness and suitability, including evaluating contraindications, allergies, dosage, administration route, frequency, and formulary status.
  • Clarifying and correcting physician medication and IV orders, as well as managing the medication reconciliation process.
  • Interpreting and rectifying outpatient prescriptions, ensuring appropriate medication selection.
  • Monitoring patients on designated target medications and initiating pharmacokinetic dosing protocols.
  • Evaluating drug therapies for suitability and providing recommendations to healthcare providers.
  • Prescribing medications in accordance with collaborative drug therapy management agreements and approved protocols.
  • Dispensing medications and providing patient counseling to ensure understanding and compliance.
  • Identifying, monitoring, and reporting adverse drug reactions and medication errors.
  • Overseeing and mentoring pharmacy technicians to ensure accurate order processing and preparation.
  • Training pharmacy interns and externs, potentially serving as a preceptor as defined by regulatory standards.
  • Contributing to the development of drug protocols and clinical pathways.
  • Conducting drug utilization evaluations on selected medications and reporting findings to the Pharmacy & Therapeutics Committee.
  • Performing additional duties as assigned.

Qualifications:
To be considered for this role, candidates should possess:

  • A Bachelor's Degree in Pharmacy or a Doctor of Pharmacy from an accredited institution.
  • Demonstrated clinical pharmacy skills and customer service abilities.
  • A valid Pharmacist license in Washington or the ability to obtain one.
  • Preferred certifications in Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support.
  • Proficiency in basic computer skills, including MS Office applications.
  • Strong organizational, planning, and prioritization skills.
  • Excellent verbal and written communication skills.
  • The ability to work independently and think critically.
  • Strong mathematical and analytical capabilities.

Working Conditions:
The role requires the ability to:

  • Move around the workplace with occasional sitting.
  • Engage in activities such as squatting, kneeling, and bending.
  • Utilize fine motor skills for precise tasks.
  • Lift and carry items up to 30 lbs.
  • Work in environments with exposure to biohazards and noise.
