This role is crucial in ensuring the effective use of medications to enhance patient outcomes and improve their quality of life.
The responsibilities associated with delivering compassionate pharmaceutical care encompass both cognitive and medication distribution tasks, including:
- Dispensing and distributing medications.
- Providing guidance to healthcare providers and patients on the correct use of prescribed medications.
- Collaborating with other healthcare professionals to plan, monitor, and evaluate patient treatment effectiveness.
- Monitoring patients for potential side effects and drug interactions.
Key Responsibilities:
- Assess medication orders for accuracy and appropriateness, considering contraindications, allergies, dosage, route, frequency, and formulary status.
- Clarify and rectify physician medication and IV orders, including the medication reconciliation process.
- Review and adjust prescriptions for outpatient dispensing.
- Monitor patients on specified target medications and initiate pharmacokinetic dosing protocols.
- Evaluate drug therapy for suitability and provide recommendations to clinicians.
- Prescribe drug therapy in accordance with collaborative management agreements and approved protocols.
- Dispense medications for administration and provide patient counseling.
- Identify, monitor, and report adverse drug events and medication errors.
- Oversee and mentor pharmacy technicians to ensure accurate order entry and preparation.
- Supervise pharmacy interns and externs, potentially serving as a preceptor as defined by the State Board of Pharmacy.
- Contribute to the development of drug protocols and clinical pathways.
- Conduct drug usage evaluations on selected medications and report findings to the Pharmacy & Therapeutics Committee, completing quality assurance audits as required.
- Perform additional duties as assigned.
Qualifications:
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited institution.
- Demonstrated foundational pharmacy clinical skills.
- Proven customer service abilities.
- Current licensure as a Pharmacist in Washington or eligibility to obtain licensure.
- Preferred certifications: Basic Life Support, Advanced Cardiac Life Support, Pediatric Advanced Life Support.
- Basic proficiency in computer skills, including MS Office applications.
- Ability to identify medications and read graduation marks on syringes and flasks.
- Strong organizational, planning, and prioritization skills.
- Excellent verbal and written communication skills.
- Capacity to think and act independently.
- Strong mathematical and analytical capabilities.
Working Conditions:
- Ability to navigate the work area with occasional sitting.
- Occasional squatting, kneeling, and bending.
- Fine motor skills necessary for precise procedures.
- Frequent reaching and lifting up to 30 lbs.
- Occasional pushing and pulling with a force of up to 14 lbs.
Environmental Conditions:
- Exposure to biohazards, body fluids, and airborne particles.
- Ability to perform tasks in a noisy environment.
Mental/Visual Requirements:
- Vision and hearing within normal limits (corrective devices permitted).
- Ability to communicate and exchange accurate information.
