Position Overview:

Key Responsibilities:
This role is pivotal in ensuring the effective utilization of medications to enhance patient outcomes and quality of life. Responsibilities include:
  • Assessing medication orders for correctness and suitability, considering contraindications, allergies, dosage, administration route, frequency, and formulary status.
  • Clarifying and correcting physician medication and IV orders, along with overseeing the medication reconciliation process.
  • Evaluating outpatient prescriptions and selecting appropriate medications for dispensing.
  • Monitoring patients on designated target medications and initiating pharmacokinetic dosing protocols.
  • Reviewing drug therapy for suitability and providing recommendations to healthcare providers.
  • Prescribing medication in accordance with collaborative drug therapy management agreements and approved protocols.
  • Dispensing medications and providing patient counseling.
  • Identifying, monitoring, and reporting adverse drug reactions and medication errors.
  • Supervising and training pharmacy technicians to ensure accurate order processing and preparation.
  • Guiding pharmacy interns and externs, potentially serving as a preceptor as defined by state regulations.
  • Contributing to the development of drug protocols and clinical pathways.
  • Conducting drug usage evaluations on selected medications and reporting findings to the Pharmacy & Therapeutics Committee.
  • Completing other assigned duties as necessary.

Qualifications:
  • Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited institution.
  • Required: Demonstrated clinical pharmacy skills and customer service abilities.
  • Required: Current licensure as a Pharmacist in Washington or eligibility to obtain licensure.
  • Preferred: Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support certifications.
  • Proficient in computer skills, including MS Office applications.
  • Ability to identify medications and interpret measurements accurately.
  • Strong organizational, planning, and prioritization skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently and think critically.
  • Strong mathematical and analytical abilities.

Working Conditions:
  • Ability to navigate the workspace with occasional sitting, squatting, kneeling, and bending.
  • Fine motor skills required for precise tasks.
  • Occasional lifting of up to 30 lbs and pushing/pulling up to 14 lbs of force.

Environmental Factors:
  • Exposure to biohazards, bodily fluids, and airborne particles.
  • Ability to perform tasks in a noisy environment.

Mental and Visual Requirements:
  • Vision and hearing within normal limits (corrective devices permitted).
  • Effective communication and information exchange abilities.
