Job Summary

We are seeking a highly skilled and compassionate Care Coach RN Case Manager to join our team at Optum. As a key member of our interdisciplinary team, you will play a vital role in coordinating care and providing ongoing nursing assessments to patients and their families.

Key Responsibilities
  • Coordinate Care: Work closely with patients, families, and healthcare providers to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct Home Visits: Perform regular home visits to assess patient needs, provide education, and coordinate care with other healthcare professionals.
  • Provide Ongoing Assessments: Conduct ongoing assessments of patient needs, including physical, functional, psychosocial, and environmental needs.
  • Assess Caregiver Needs: Identify caregiver burnout and provide resources and support to help caregivers manage their role.
  • Implement Care Plans: Develop and implement individualized care plans that address patient needs and goals.
  • Collaborate with Interdisciplinary Team: Work closely with other healthcare professionals, including physicians, social workers, and therapists, to ensure comprehensive care.
  • Document Progress: Accurately document patient progress and care plans in the electronic health record.
  • Stay Up-to-Date: Stay current with industry developments and best practices in care coordination and nursing.
Requirements
  • Registered Nurse: Current RN license in good standing.
  • Experience: 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
  • Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic health records.
  • Education: Bachelor's degree in Nursing or related field.
Preferred Qualifications
  • Hospice and Palliative Care Experience: Experience working in hospice and palliative care settings.
  • Home Visit Experience: Experience conducting home visits and providing care in the home setting.
  • Language Skills: Bilingual or multilingual skills, particularly in Spanish.
Physical and Mental Requirements
  • Ability to Lift: Ability to lift up to 100 pounds.
  • Ability to Sit and Stand: Ability to sit and stand for extended periods of time.
  • Ability to Use Fine Motor Skills: Ability to use fine motor skills to operate office equipment and machinery.
