Position Overview:
The Registered Nurse Case Manager is responsible for coordinating comprehensive care and conducting initial ongoing nursing assessments for patients and their families. This role involves collaborating with interdisciplinary team members to develop and implement individualized care plans.

Key Responsibilities:

  • Conduct in-home medical assessments for established patients, ensuring hands-on evaluations during each visit.
  • Perform urgent assessments to prevent hospital admissions, focusing on delivering optimal care in the home environment.
  • Continuously evaluate the impact of chronic illnesses on patients' physical, functional, psychosocial, and environmental needs.
  • Identify caregiver burnout and provide appropriate community resources.
  • Implement and modify care plans based on ongoing assessments and patient needs.
  • Execute nursing procedures, including wound care, phlebotomy, and injections.
  • Facilitate advanced care discussions with patients and families, documenting their preferences in the electronic medical record.
  • Initiate preventive and rehabilitative nursing interventions as necessary.
  • Assess patient and family needs throughout the care continuum.
  • Evaluate patient responses to care and adjust plans accordingly.
  • Participate actively in interdisciplinary meetings, contributing valuable insights.
  • Utilize problem-solving skills to address challenges and improve processes.
  • Conduct telephonic follow-ups and case management as required.
  • Serve as a resource for other nursing staff.
  • Engage in ongoing education to stay current with chronic medical conditions.
  • Build trusting relationships with patients, caregivers, and healthcare team members.
  • Collaborate with providers to enhance care standards and policies.
  • Maintain professionalism and courtesy in all interactions.
  • Document patient progress in the electronic health record.
  • Manage a patient caseload, ensuring timely visits and documentation.
  • Perform additional nursing duties as assigned.

Qualifications:

  • Active Registered Nurse license in the state of employment.
  • Current BLS certification or ability to obtain within a specified timeframe.
  • Minimum of three years of experience in a clinical setting.
  • Demonstrated nursing skills and proficiency in computer applications.
  • Ability to work effectively within multidisciplinary teams.
  • Reliable transportation for patient visits.

Preferred Qualifications:

  • Certification in Hospice and Palliative Care Nursing.
  • Experience in hospice care and home visitations.
  • Knowledge of advanced care planning and end-of-life discussions.
  • Bilingual proficiency in English and Spanish.

Physical and Mental Requirements:

  • Ability to lift and transport patients as needed.
  • Capacity to sit or stand for extended periods.
  • Strong problem-solving and logical reasoning skills.
