Nursing Care Coordinator

As a Nursing Care Coordinator, you will play a pivotal role in assessing and managing the healthcare needs of patients and their families. Collaborating with an interdisciplinary team, you will ensure the effective implementation of personalized care plans while fostering strong relationships with patients and caregivers.
Key Responsibilities:
- Conduct comprehensive home assessments for established patients, ensuring thorough evaluations during each visit.
- Provide urgent care interventions aimed at preventing hospital admissions, delivering high-quality medical support in the home environment.
- Continuously assess the impact of chronic illnesses on patients' overall well-being, addressing physical, emotional, and social needs.
- Identify caregiver stress and connect families with appropriate community resources.
- Develop and modify individualized care plans based on ongoing assessments and patient feedback.
- Perform essential nursing procedures, including wound care, phlebotomy, and injections.
- Engage in advanced care discussions with patients and families, documenting their preferences in the electronic health record.
- Initiate preventive and rehabilitative nursing actions as necessary.
- Maintain consistent evaluation of patient and family needs throughout the care continuum.
- Participate actively in interdisciplinary meetings, contributing valuable insights to enhance patient care.
- Utilize problem-solving skills to address challenges, applying established policies and practices.
- Conduct follow-up calls and telephonic case management as required.
- Serve as a resource for nursing assistants and other team members.
- Engage in ongoing education to stay current with chronic medical conditions and fulfill licensing requirements.
- Build trusting relationships with patients, families, and healthcare providers.
- Collaborate with providers to refine policies and standards that impact patient care.
- Exhibit professionalism and courtesy in all interactions with patients and colleagues.
- Communicate patient needs and care outcomes to providers in a timely manner.
- Coordinate services and prioritize patient needs in collaboration with the interdisciplinary team.
- Document progress toward care goals in the electronic health record.
- Employ a case management approach, referring to additional services as needed.
- Manage a patient caseload, ensuring timely visits and documentation.
- Perform other nursing duties as assigned.

Required Qualifications:
- Active Registered Nurse license in the state of employment.
- Current Basic Life Support (BLS) certification or willingness to obtain within 30 days of hire.
- Minimum of 3 years of experience in a clinical setting, such as a physician's office, hospice, or hospital.
- Demonstrated proficiency in nursing skills.
- Strong computer skills, including proficiency in Microsoft Office applications.
- Ability to work effectively with diverse teams and individuals.
- Reliable transportation for travel to various locations as needed.
- Valid driver's license in the state of employment.
Preferred Qualifications:
- Certification in Hospice and Palliative Care Nursing.
- Experience in hospice care or community-based nursing.
- Familiarity with advanced care planning and end-of-life discussions.
- Bilingual proficiency in English and Spanish.
- Exceptional verbal and written communication skills.
- Strong organizational and prioritization abilities.
Physical & Mental Requirements:
- Ability to lift up to 100 pounds and push or pull heavy objects.
- Capability to sit or stand for extended periods.
- Proficient use of fine motor skills for operating office equipment.
- Ability to comprehend and follow verbal and written instructions.
- Logical reasoning skills for problem-solving.
