Case Manager II: $43,305.60 Annually

Case Manager III: $46,009.60 Annually

Employment Type: Full-time

Key Responsibilities:

  • Maintain professional and ethical communication using Trauma-Informed Care principles.
  • Act as a Family Support Specialist within the Family and Youth Success Program, delivering direct support to families and youth.
  • Conduct screening and assessments, provide crisis intervention, implement evidence-based programs, and facilitate skills training for families and individuals.
  • Deliver services across various environments, including home, educational institutions, and community settings.
  • Provide services primarily in designated regions and be open to travel as required.
  • Comply with the organization's Code of Conduct and report any ethical or business concerns.

Qualifications:

  • Bachelor's degree from an accredited institution.
  • At least one year of experience working directly with at-risk youth and families.
  • For Case Manager II, a minimum of one year in behavioral health case management is required.
  • For Case Manager III, a minimum of two years in behavioral health case management is necessary.
  • Graduate studies in a relevant field may substitute for up to one year of experience.
  • Possession of a valid driver's license and insurability under agency policies is mandatory.
  • Applicants must be at least 18 years old.

Preferred Skills:
