Position Overview

This role involves evaluating physician prescriptions to ensure the accurate administration of medications to patients across all age groups, from infants to seniors, ensuring that the most suitable drug and dosage are prescribed, dispensed, and monitored effectively.

Key Responsibilities:
(30%) Assess and verify all prescriptions for accuracy and appropriateness, dispensing the most suitable medications for each patient.
(30%) Oversee the preparation of intravenous solutions and other parenteral products.
(30%) Manage pharmacy operations during shifts, ensuring effective communication and problem resolution between shifts.
(10%) Engage in collaborative patient care rounds with nursing staff, physicians, dietitians, social workers, and other healthcare professionals to adopt a multidisciplinary approach to patient care.

Certifications:
CSL PHARM – Controlled Substance
PHARM – Pharmacist
Education:
Bachelor's Degree
Completion of a 6-year Pharm D. program is preferred.

Additional Responsibilities:
Provide and document pharmaceutical information to healthcare professionals and patients.
Assist in maintaining the hospital formulary by addressing non-formulary orders and managing pharmacy inventory.
Advise on antibiotic usage, transitioning patients to oral therapies, and identifying duplicate therapies.
Participate in the education of patients, students, and other healthcare professionals through discharge counseling, in-services, and hands-on training.
Contribute to the training of new pharmacists and pharmacy interns and residents. Engage in the development of new or revised pharmacy policies and procedures through active participation in performance improvement teams.
Manage dosing for medications provided by the kinetics service and adhere to kinetic policies and procedures. Oversee the dosing of renally eliminated drugs and Total Parenteral Nutrition (TPN).
Prepare IV and piggyback medications, TPNs, and narcotic infusions, ensuring compliance with aseptic techniques and universal precautions as per USP 797 guidelines.

Skills and Experience:
Basic computer skills are required for entering prescription information.
Strong organizational skills are necessary to prioritize multiple tasks effectively.
Excellent interpersonal skills are essential for successful communication with individuals and groups at all levels, conveying ideas and concepts clearly and understandably.
Experience of one to three years in a hospital or medical center pharmacy setting is preferred.
