About the Role

We are seeking a highly skilled and compassionate Patient Problem Solving Specialist to join our team at LA Family Housing. As a key member of our Homeless Services team, you will play a critical role in coordinating efforts within the Coordinated Entry System (CES) to ensure seamless transitions of care for individuals experiencing homelessness.

Key Responsibilities
  • Cultivate Strong Relationships: Develop and maintain strong, collaborative relationships with hospital partners, community providers, and internal departments to ensure effective communication and coordination of care.
  • Train and Educate: Provide training and education to hospital staff on access to resources within CES and other relevant resources for individuals experiencing homelessness.
  • Liaison and Problem-Solving: Serve as a liaison between hospitals and community providers, participating in hospital case conferencing and collaborating in problem-solving efforts for individuals experiencing homelessness.
  • Technical Support: Provide on-call technical support to hospital staff as needed, ensuring timely and effective resolution of issues.
  • Tracking and Referral: Assist in tracking participants' homeless service connections and re-connecting to providers as necessary, referring patients experiencing homelessness to CES Access Sites for assessment and service connection.
  • Documentation and Reporting: Maintain accurate and timely records of participant interactions, using clinical documentation and HIPAA standards, and track metrics as assigned by supervisor.
Requirements
  • Knowledge and Experience: Possess knowledge of homeless populations, systems, and programs, specifically the Coordinated Entry System (CES), and have at least two years' experience in a system coordination role, preferably in a hospital setting or healthcare patient advocacy role.
  • Skills and Abilities: Demonstrate ability to work independently and as part of a team, build relationships with stakeholders, present information thoroughly to a range of stakeholders, and demonstrate creative and solution-focused problem-solving skills.
  • Technical Skills: Be proficient in Microsoft Programs (Word, Excel, Access, PowerPoint), Zoom, and web-based software.
