Social Services Coordinator

We are seeking a highly motivated and experienced Social Services Coordinator to join our team at Retirement Housing Foundation. The successful candidate will be responsible for assessing and identifying residents in need of services, coordinating the delivery of services, and establishing and maintaining linkages with service providers in the community.
Key Responsibilities- Assess and identify residents in need of services and coordinate delivery of services.
- Establish and maintain linkages with service providers in the community.
- Develop and maintain ongoing educational health and wellness programs for residents.
- Develop and maintain a comprehensive list of available community resources for residents.
- Serve as resident advocate and teach residents to advocate for themselves.
- Serve as liaison to community agencies, network with community providers, and seek out new services available to residents.
- Maintain positive relationships with fellow residents to ensure that communication between residents and community service providers is open and effective.
- Serve as advocate/liaison with management, other departments of the housing facility, and outside agencies who provide social services, health benefits, training, and other services for seniors to Age in Place.
- Assist with providing information and referrals for crisis intervention and conflict resolution.
- Monitor resident referrals with agencies.
- Maintain accurate and up-to-date records to include daily and monthly reports.
- Maintain resident files in compliance with HUD requirements.
- Working knowledge of supportive services and community resources.
- Ability to advocate and problem solve.
- Must uphold ethical standards and client confidentiality.
- Must work well with population served, staff, and partners.
- Ability to work with limited supervision.
- Excellent communication skills; written and verbal.
- Excellent time management skills.
- Experience in MS Office Suite: Word, Excel, PowerPoint, and Outlook.
- Bachelor's degree in Social Work, Gerontology, Human Services, or Sociology.
- Candidates with appropriate work experience will be considered.
- Experience working with a diverse group of seniors, children, families, and disabled individuals.
- Spanish speaking a plus.
