Status: Full Time (40 hours per week)

Work Schedule: Mondays through Fridays 8:30 am to 5 pm

GENERAL SUMMARY

The Community Connector will perform community outreach based on referrals received from community contacts about victims of non-fatal shootings and persons at risk of perpetuating a cycle of violence in North Philadelphia and engage those persons together with APM's Violence Intervention Therapists.


Some responsibilities and duties:

  • In collaboration with the APM Violence Intervention team, provide community engagement and in the 24th and 26th Philadelphia Police Districts under the direction of the Program Director and community leadership
  • Outreach to non-fatal shooting victims as identified by Police Department, ideally within 48hrs of an incident occurring
  • Working with Hospital Based Violence Programs to engage with and deem eligibility for non-fatal shooting victims in their care
  • Meet varying criteria for different services offered to each young person deemed eligible within a specific time-frame—i.e. mental health screening, safety planning meetings, healing circles, etc.
  • Carrying a small caseload when necessary of non-fatal shooting victims
  • Represent APM as appropriate in community events, meetings, learning communities in coordination with other APM Program Staff
  • Collaborate with APM Directors to engage with key partners to increase referrals, resources and supports for participants
  • Maintain the Non-Fatal Shooting Victim Outreach list which will range from 20-30 young people depending on capacity
  • Collaborate with APM Directors to design plans to engage young people on a weekly basis. This will include off-track young people, red flags who have been difficult to locate, young people who have not been attending programming and those who may need intentional conversations around safety issues
  • Maintain weekly contact with young people who are on the Non-Fatal Shooting Victim Outreach list that have not yet been deemed eligible for a TR-Caseload
  • Locate young men who have been newly identified as non-fatal shooting victims by the Police Department
  • Perform initial eligibility assessments and vetting procedures for those on the Non-Fatal Shooting Victim Outreach List
  • Work with supervisor to gather new information from partners for locating young men

Requirements:
  • EDUCATION: BA preferred/Equivalent experience accepted - Minimum of three years of direct service work with high-risk young people
  • EXPERIENCE: Understanding and experience working with street involved high risk young people
  • Valid Driver's License or access to public transportation
