WHAT YOU WILL DO:

The Intake Coordinator is a non-exempt position? responsible for admitting and coordinating initial services to the Hub clients.

  • Ensure clients are admitted into the Hub on the same day as arrival for service.
  • Complete all intake paperwork comprehensively and make referrals to additional services, as necessary.
  • Run reports in electronic health record system as required.
  • Participate in trainings and team meetings.
  • Maintain contact with clients through routine calls and follow-ups.
  • Complete client satisfaction surveys and track data for outcomes.
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:
  • Bachelor's degree in a human services related field preferred
    • or Bachelor's degree from an accredited college/university and two (2) years' experience working in a human services related field.
  • Must have a valid drivers' license and auto insurance coverage.
