Job Overview

Position Summary:
The Client Engagement and Support Coordinator will implement a holistic approach to assist individuals who have transitioned from temporary to stable housing, fostering self-sufficiency and community integration over a one-year period.

This role involves managing all intake processes and admissions for the HERO program, conducting meetings with prospective clients, and collecting necessary documentation.

The Coordinator will provide ongoing case management for retention clients for up to one year, ensuring they achieve sustained success within the community.

Additionally, the Coordinator will support clients in maintaining familial and community connections, collaborating with the clients, Financial Stability Counselor (FSC), and Substance Use Counselor (SUC) to monitor the effectiveness of the Individual Service Strategy/Treatment Plan of Care (ISS), and coordinate tailored services for housing placement, including the involvement of community-based providers, while meticulously documenting all related activities and services.
