OUR MISSION


The mission of Queen of Peace Center is to provide family-centered care for women with substance-use disorders, their families, and at-risk youth through treatment, prevention and education and housing.

POSITION DESCRIPTION


The Intake Coordinator is responsible for providing a client centered, welcoming and compassionate environment for clients seeking admission to Queen of Peace Center.

This individual is responsible for answering phone calls, providing information about agency services, availability and community resources and accurately registering new clients into the EHR and state billing system.

The Coordinator assists the clients during the intake process, working with multiple departments to ensure the process flows smoothly, all information necessary for registration and annual review is captured and thoroughly and accurately documented and clients are smoothly linked to their treatment providers.

Job Responsibilities

  • Responsible for assisting in the delivery of services to clients according to approved policies and procedures meeting DMH, CSTAR and COA standards.
  • Maintain clients' charts, records and files in accordance with COA, DMH and CSTAR standards.
  • Work as a team player in the pre-admission process, including assisting in the collection of all data to be processed and finalized during the admission process.
  • Serve as a liaison between clinical staff and clients during the intake process.
  • Schedule client appointments as needed
  • Answer incoming phone calls and return voicemails same day as they are received, providing information
  • Verify and document all appropriate information into the EHR system and state system as directed.
  • Maintain accurate census information in billing system and document wait list in state system as directed.
  • Assist in communicating with outside agencies, such as treatment centers, AA, Catholic Charities' agencies and other resources to meet client needs via telephone, email and in person.
  • Pursue continuing professional development, join appropriate professional groups, and attend pertinent meetings.
  • Provide information about provider services,
  • Perform other duties as assigned within the scope of the position.
  • Some evenings and weekends may be required.

Job Requirements

  • High school diploma required
  • Experience working in an EHR preferred
  • Experience in working in a behavioral healthcare environment preferred
