As a TBI Service Coordinator, you would be responsible for: · Providing service coordination to individuals and families residing in the community at large · Maintaining current knowledge base of DOH policies, procedures and regulations related to service coordination · In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of life TBI Service Coordinator Minimum Salary: $20.51/hour TBI Service Coordinator Maximum Salary: $23.07/hour TBI Service Coordinator Schedule: M-F 8:30am-5:00pm; Combination of in-office and remote work Qualifications: · Valid NYS Driver’s License · Experience is required. You must: o (A) Be an individual with a Master’s degree and one (1) year experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o (B) Be an individual with a Bachelor’s degree and two (2) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o (C) Be an individual with an Associates degree and three (3) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o (D) Be an individual with a High School Diploma or equivalent (GED) with four (4) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o (E) Be an individual who has successfully served as a Regional Resource Development Specialist for one (1) year in the HCBS/TBI Waiver Program.
