Responsibilities:

  • Conducts pre and post care coordination and case management services for patients seen by the DBP service providers or the Clinical Psychologist II.
  • Reviews and maintains waitlists for referrals for assessments to be completed by DBPs or Clinical Psychologist II.
  • Prioritizes referrals according to severity and level of need.
  • Ensures appropriate intake steps are followed, including insurance eligibility & authorization, assessment of needs, collecting patient data, enrolling in programs, developing case plan, and other steps as required by department guidelines.
  • Compiles information according to specialist requests prior to appointment for initial interview.
  • Works with child's caregivers to identify barriers related to attending services and following the case plan according to post assessment provider recommendations and caregiver priorities. Conducts initial and ongoing assessment of family's needs for support and resources through the time that the family engages successfully in recommended services.
  • Functions as first respondent after the medical provider refers for assessment/testing. Schedules up to 3 consecutive appts. per child to be assessed (for initial caregiver interview, clinical assessment, and debriefing about recommendations and plan/next steps.
  • Tracks attendance and follows-up with patients who missed one of the appts. and reschedules to ensure completion of assessment process. Assesses families' need for resources to minimize barriers to attend the assessment and/or recommended services.
  • Facilitates care transitions between providers and follows-up on referrals within established timeframes.
  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
  • Performs other duties as assigned.

Requirements:

  • 2 years of work experience providing human services to high risk, medically underserved, children with disabilities, or relevant populations required.
  • Bachelor's degree in child development, social work, psychology, or closely related field required.
  • Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
  • Ability to work well in both a team-based environment and independently.
  • Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
  • Basic coaching skills, such as reflecting, active listening, and paraphrasing; knowledge of motivational interviewing desirable.
  • Basic organizational skills, attention to detail, time-management skills, and motivation to meet deadlines and achieve goals.
  • Bilingual in English/Spanish required.
  • Demonstrated ability to be culturally sensitive and respect diversity.
  • Excellent interpersonal and customer service skills.
  • Excellent written and verbal communication skills.
  • Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.

$22.00 - $27.47

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
