San Diego Youth Services seeks a Bilingual Case Manager in the Safe Family Services Program to be based at College Heights office location. This is a full time & non-exempt position reporting to the Safe Family Services Program Manager.

San Diego Youth Services has helped improve the lives of more than 780,000 homeless, runaway and vulnerable youth since its beginning in 1970. We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing to more than 28,000 youth every year at 100+ community and school locations. Through prevention, early intervention and treatment, we also help youth before they need higher levels of care or become homeless.

  1. Meet the basic needs of youth. Hunger, poverty and youth homelessness are on the rise for many youth and their families today. We help meet these basic needs for youth. What could be more vital than food and shelter? We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing.
  2. Create positive connection. We help youth focus on positive life changes and healthy relationships as part of providing needed resources and services. San Diego Youth Services has long provided foster homes and adoption support services for families considering adopting a youth in the foster care system.
  3. Help youth develop life skills. In addition to a range of safe and stable housing options, we offer support to help youth develop the independent living skills necessary to become self-sufficient and keep off the streets.
  4. Promote overall health and well-being. Early intervention and care for the whole child are critical. We provide a broad continuum of services to help youth overcome trauma and challenges that put them at risk, including case management, counseling, mental health treatment and educational support and resources.

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR

Our Safe Family Services Program offers supportive services to Transition Age Youth (TAY), ages 18-24 years old, currently in an SDYS program, who have, historically, or are currently experiencing domestic violence.


CONTEXT OF THE POSITION


The Case Manager will provide comprehensive trauma-informed advocacy services with the CSEC Response Team through the Safe Family Services Program, including, but not limited to, working in collaboration with Child and Family Wellbeing, and other community agencies in addition to providing services to youth who are adolescents and Transitional Aged Youth who have experienced commercial sexual exploitation, human trafficking, and/or Domestic Violence. The Case Manager is responsible for screening intakes, providing intake assessments, creating a service plan in partnership with the youth, providing crisis intervention, facilitating prevention and intervention groups, conducting outreach activities, providing advocacy and assisting in the day-to-day programmatic functioning.


Essential functions include:


  • Provide support services that include accompanying youth to meetings/appointments/court; case management services around stabilization (i.e., housing, employment, education, health, etc.); outreach activities, crisis intervention, and referrals as applicable. Provide transportation when needed.
  • Maintain accurate and timely documentation, data collection and contract compliance of records including file reviews.
  • Conduct emergency and/or immediate outreach in collaboration with Child and Family Wellbeing to prospective service partners in a variety of community settings (Polinsky, Group Homes, Juvenile Hall, In-home, Community Partners, etc.); screen prospective service partners for eligibility to enter program. Triage with CSEC Response Team Coordinator for needed services.
  • Assist in the development and maintenance of group curriculum and facilitate groups.
  • Ensure coordination of care by communicating with Probation Officer, CWS Social Workers, Health Care Providers, Therapist, Natural Support Systems, Parents/Families, etc. to gain knowledge of service-partners and how to best ensure their success.
  • Ability to be on-call. A typical on-call rotation is 1 week every 5-6 weeks and is subject to change.
  • Provide intensive 24hr follow-up on all referrals.
  • Engage youth in services for up to 90 days or longer, depending on client need and linkage to long-term resources, of intensive short-term case management.
  • Provide outreach and training to increase community awareness.
  • Knowledgeable or willing to be trained in tenant-landlord laws, Fair Housing Laws, and the Transitional Housing Misconduct Act.
  • Seek out new partnerships with low-income housing providers.
  • Provide transportation when needed.
  • Perform other duties as assigned.


PREFERRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS


  • Bachelor's degree in social work or related field or 2-5 years’ experience working with high-risk clients, preferably adolescents and Transitional Aged Youth who have experienced commercial sexual exploitation, human trafficking and/or Domestic Violence.
  • Preferred lived experience through being a Survivor of victimization such as human trafficking, domestic violence, sexual abuse or experience as a former foster youth.
  • Bilingual required
  • Experience in crisis intervention, group facilitation, documentation, and case management preferred.
  • Must possess excellent oral and written skills; work well autonomously and with multi-disciplinary team.
  • Willingness to develop or possess working knowledge of trauma-informed care.
  • Must have or be willing to be trained 40-hour Domestic Violence counselor training, Campbell Danger Assessment, Mental Health First Aid, Seeking Safety, and Motivational Interviewing (MI) trainings.
  • Must be computer literate in Microsoft Outlooks, Word and Excel.
  • Valid CA driver’s license, good driving record and willingness to use own properly insured vehicle on the job (mileage reimbursed).
  • Maintain certification in CPR and First Aid.
  • Employment is subject to required clearances which include tuberculosis test, fingerprinting, background checks (FBI, DOJ, CACI), and drug testing.

Salary Range: $21-23/hour + up to $1500 annual bilingual differential based on proficiency
