Position: Cal OES Case Manager

Classification Non-Exempt

Supervised By: Housing Program Manager

Home Start’s vision is for every child to have a safe, stable, and nurturing home. Guided by our mission to assure the safety and resiliency of children by strengthening families and their communities, Home Start, Inc., a non-profit child abuse prevention, and treatment agency has strengthened and developed San Diego’s Families and communities since 1972.

Home Start, Inc. operates programs designed to prevent homelessness, interrupt the cycle of homelessness, and promote self-sufficiency. Our programs are designed to ensure homelessness in San Diego is rare, brief, and non-recurring.

Job Summary: Provides case management services and support to transitional housing clients in the CalOES XH program.

Duties and Responsibilities:

  1. Coordinate with referral sources and respond within 48 working hours.
  2. Conduct client assessments consistent with program requirements.
  3. Adhere to Home Start principles and practices to include Trauma Informed Care; Harm Reduction and Safety Planning; Mobile Advocacy and Flexible Engagement.
  4. Provide case management services including, but not limited to: identification of client’s needs and development of client’s individualized housing stability plan with ongoing monitoring and follow-up on progress towards those goals for safety and self-sufficiency.
  5. Assist client actively look for long-term stable housing options.
  6. Help participants establish linkages to community services and resources that help them achieve their goals.
  7. Work with the residents helping them understand and navigate through various systems including child welfare, social services, and juvenile justice (i.e. sealing juvenile delinquency and/or adult criminal records).
  8. Help residents establish and strengthen their support network of peers, family and community members as well as to self-help groups.
  9. Encourage participants to make choices that reduce risk and promote safety.
  10. Complete and maintain client records that document services provided, with weekly case management entries into Clarity system.
  11. Perform required follow-up checks with alumni clients.
  12. Participate in system data collection; complete necessary paper work and forms.
  13. Flex schedule to work occasional weekend days, if necessary.
  14. Other duties as assigned.

Qualifications:

Education A Bachelor’s Degree in human services or related field or equivalent work or volunteer experience and training required. Skills: Must work well with persons of varied sexual orientation, racial, ethnic, cultural, socioeconomic backgrounds, and with individuals who are diverse. Previous employment or volunteer work with low-income, unhoused, or disadvantaged youths in shelter or residential settings preferred. Knowledge of family systems, community resources, substance abuse treatment, and interventions required. Good oral and written communication and computer skills are required. Bilingual (Spanish/English) speaker preferred. Other Requirements: Must pass a criminal background check. Must have a valid California driver’s license, a safe driving record, automobile insurance, and a reliable vehicle.

Physical Demands:

Reading and typing on a computer frequently.

  • Sitting for long periods of time at required meetings or in supervision.
  • Daily traveling in car to meet with clients and attend agency and community meetings.

Work Environment:

Office space may be shared with frequent telephone and visitor contacts.

  • The noise level fluctuates from quiet to noisy.
  • Travel to meet with clients and community resource contacts.
