About La Raza Community Resource Center:
La Raza Community Resource Center (LRCRC) is a bilingual, multi-service non-profit organization dedicated to meeting the social service, immigration, educational, and leadership development needs of low-income families and individuals. La Raza Community Resource Center is located in the heart of the historic San Francisco Mission District, the vibrant cornerstone of San Francisco's Latino community. We have been serving the community for over 50 years. Our Social Services Program services include a food pantry, family counseling, educational workshops, support groups, rental assistance, and many other services. Our Immigration Program attorneys provide both affirmative family-based immigration services and removal defense. All of our legal and social services are free (subsidized).
Job Summary:
The Family Services Specialist is passionate about working with immigrant families and adopts a strengths-based, non-judgmental approach. This job requires the ability to build meaningful relationships with a diverse range of individuals and agencies. It also requires a deep understanding of the unique challenges faced by these families, the ability to offer trauma-informed case management, and the experience to provide a range of community and family support services. The Family Services Specialist is responsible for ensuring that the standards of quality for family strengthening and support framework are implemented. This includes conducting a comprehensive needs/strengths assessment and service plan, monitoring the progression of the service plan, and assessing the quality of the social services provided to families.
Responsibilities:
- Provide trauma-informed and person-centered case management support that includes consistent engagement to families, at least two contacts a month but preferably weekly, assisting families in developing the skills and knowledge to eventually access resources independently.
- Completes formal intake, and needs assessment, and facilitates planning process to assist families in developing a plan of action to address concerns impacting child development and family functioning.
- Maintains confidentiality of all family records, organizes case file documentation, and completes daily family contact/comprehensive notes according to procedures.
- Conduct intake, written assessment, case notes, individual contact, referral log, service plan, and consistent monitoring to review progress towards identified goals. Services can be short or long-term, the last being at least 6 months or longer.
- Provides and documents long and short-term interventions by providing information, referrals, and resources. This includes connecting families to other LRCRC programs and community resources.
- Collaborate with other agencies, advocating on behalf of families to coordinate care and follow up to ensure the families accessed the services or located a different option.
- Completes all required reports, forms, and documentation for family and community partnerships timely and consistent.
- Keep detailed case records timely and by program and agency standards and deadlines, such as sign-in sheets, evaluations, enrollment forms, client satisfaction surveys, assessments, service plans, case notes, and incident reports.
- Works in tandem with the parents, staff, and community representatives to coordinate family services and meet all Standards of Quality for Family Strengthening and Support Framework
- Participates in scheduled staff meetings, in-service, and training.
- Meet with the Director of Family Resources on a weekly basis to report, problem-solve, and plan regarding position responsibilities.
- Ability to interact positively with team members, and facilitate positive change.
- Partner with the supervisor when necessary to address department issues or concerns.
- Other tasks as assigned.
Qualifications:
- Experience supporting families overcoming barriers, such as housing, immigration, child safety, family functioning/relationships, parenting, health, mental health, substance abuse, socio-economics, child school success, and/or well-being.
- Stress Management - Ability to practice self-care habits and emotional regulation to notice earlier signs of burnout and communicate with supervisor.
- AS Degree with an emphasis in Social Work, Counseling, Psychology, Human Services, Family Development, Family Studies, or 3 years of relevant experience
- Bilingual English/Spanish fluency, both oral and written.
- 3+ years’ of community-based direct service experience 1+ years of Case Management Experience
- Proficiency in Microsoft Office applications: Outlook, Word, Excel, and PowerPoint.
