Transition Specialist

As a Transition Specialist at our Public Administration organization, you will play a critical role in ensuring the successful re-entry of incarcerated individuals into society. You will be responsible for developing and implementing transition plans that address the unique needs of each individual, including housing, employment, and healthcare. Your work will directly contribute to reducing recidivism rates and promoting public safety.
Minimum Qualifications:
- Bachelor's degree in social work, criminal justice, or a related field
- Experience working with incarcerated individuals or in a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
- Master's degree in social work, criminal justice, or a related field
- Experience developing and implementing transition plans
- Knowledge of community resources and services
- Experience working with diverse populations
- Bilingual in English and Spanish
- Develop and implement individualized transition plans for incarcerated individuals
- Collaborate with community organizations and employers to identify housing and employment opportunities
- Provide counseling and support to individuals during the transition process
- Maintain accurate records and documentation of transition plans and progress
- Stay up-to-date on best practices and research related to re-entry and transition services
Skills:
As a Transition Specialist, you will utilize your strong communication and interpersonal skills to build relationships with incarcerated individuals and community partners. Your organizational and time management skills will be essential in developing and implementing individualized transition plans. Your knowledge of community resources and services, as well as your experience working with diverse populations, will enable you to identify and address the unique needs of each individual. Additionally, your ability to work independently and as part of a team will be critical in ensuring the success of our transition services.
