About the Role

This is an exciting opportunity to join our team as a Healthcare Director, leading the development and implementation of transitional care programming and supporting the delivery of high-quality care to vulnerable populations. You will work closely with hospital and Central Office leadership, ED Care Management, ED Social Work, inpatient Transitional Care Management staff, and community-based staff to ensure effective collaboration, coordination, integration, and management of care planning and delivery across disciplines.

You will analyze complex operations, identify workflow challenges, and develop solutions to improve patient outcomes and reduce preventable adverse events. Your expertise in EPIC, data analytics, visualization, quality improvement, and project management will enable you to drive process improvements, optimize workflows, and enhance patient care.

This is a full-time position, with a competitive salary of $95,000 - $125,000 per year, depending on experience, as well as a comprehensive benefits package that includes comprehensive health benefits, retirement savings, and loan forgiveness programs.
