Care and Support Specialist

We are seeking a compassionate Care and Support Specialist to join our team at Love146 Inc. in Connecticut.

The estimated salary for this position is $55,000 - $57,000 Annually, depending on experience.

About the Job:

Overview

The Care and Support Specialist will work closely with participants ages 16 and above in the Connecticut Survivor Care Program to help them achieve their goals as they prepare to transition into adulthood.

This role involves providing direct services to program participants to help them achieve their Care Plan goals.

The specialist will liaise between program participants, educational institutions, employers, housing resources, childcare resources, and other government and community-based agencies to support program participants in achieving their Care Plan goals.

Key Responsibilities:

  • Implement Love146's Survivor Care program's Transitional Services programming services with a standard of excellence.
  • Help participants to identify individual Care Plan goals as well as measurable objectives for obtaining those goals.
  • Provide direct services to program participants to help them achieve their Care Plan goals.
  • Liaise between program participants, educational institutions, employers, housing resources, childcare resources, and other government and community-based agencies.
Requirements

To be successful in this role, you should have:

  • A Bachelor in a related field, such as human resources, social work, business, or education preferred; or Associate in a related field + 2-3 years of related work experience.
  • Prior transitional services (e.g., workforce development, job placement, post-secondary education, and/or housing placement) experience.
  • Experience working directly with adolescents and young adults, preferably with high-risk populations.
  • Valid driver's license, a clean driving record.

Work Environment

You will be working in a fast-paced environment with a variety of settings, including client's homes/placements, community settings, Love146's headquarters office, and automobile travel.

Please note that this role requires intense mental and visual concentration and eye-hand coordination due to extended periods of sitting, using office equipment, and performing computer operations.
