We are seeking a skilled Registered Nurse to join our team at Yale New Haven Health. As a Transitional Youth Health Specialist, you will play a crucial role in providing high-quality patient-centered care to our patients.

Responsibilities
  • The RN will be responsible for upholding the YNHHS mission, vision, and values, as well as adhering to the Connecticut Nurse Practice Act, American Nurses Association Scope and Standards for Nursing Practice, and the YNHHS Professional Practice Model.
  • As an integral member of the healthcare team, the RN will communicate effectively with patients, families, and healthcare providers to ensure seamless care delivery.
  • The RN will prioritize patient safety, respect patient autonomy, and maintain accurate and complete records.
  • Responsibilities also include assessing patients' health status, developing personalized care plans, implementing evidence-based interventions, and evaluating patient outcomes.
Requirements

To be considered for this role, applicants must possess a valid RN license in the State of Connecticut, current BCLS certification, and a minimum of 1 year of experience in a related field.

A Bachelor's degree in Nursing (BSN) or higher is preferred, and clinical specialty certification is highly encouraged.

Compensation and Benefits

Salary: $85,000 - $115,000 per year, depending on experience. Additional benefits include:
