Job Description
MetroPlusHealth is committed to delivering exceptional healthcare services to our members. We are seeking an experienced Care Coordination Specialist - Transitions of Care to join our team and help us achieve this goal.

Key Responsibilities:
  • Provide care management support during transitions of care
  • Complete transitions of care assessments to identify member needs
  • Develop and implement individualized plans of care with the care team
  • Collaborate with members and caregivers to address gaps in care and social determinants of health issues
  • Link members to available resources and educate them on relevant chronic diseases, preventive care, medication management, and home safety
  • Ensure access to care and reduce unnecessary hospitalizations through appropriate referrals to community supports

Estimated Salary: $85,000 - $110,000 per year
