KNWM River is seeking a Service Coordinator to connect residents with services that will allow elders to live independently and age in place.

Schedule:

Part time 20 hours/week, at River Apartments in Anoka.

Responsibilities:

  • Provide general case management (including intake) and referral services to our elder subsidized housing residents who need such assistance.
  • Monitor the ongoing provision of services from community agencies and update case managers and provider agencies as appropriate.
  • Link our elder residents to the supportive services necessary for them to remain independent and in their own apartments.
  • Maintain involvement in external community activities that relate to Service Coordinator functions, coordination with community resources, and move out placement as needed.
  • Recommend in-home care for residents and work with the outside providers to provide necessary referrals.
  • Educate residents on service availability, application procedures, etc., coordinating workshops or informational speakers as appropriate.
  • Help residents build informal support networks with other residents, family, and friends to combat isolation.
  • Demonstrate professional and courteous conduct with all residents, family members, visitors, vendors, and co-workers.
  • Complete required reports to HUD, Housing Manager, and others as required.


Qualifications:

Education: Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable; a college degree is acceptable or equivalent work experience. Compliance with HUD Continuing Education requirements for Service Coordinators: must have met a minimum of 36 training hours before hiring OR complete minimum training requirements within 12 months of initial hiring.

Required Experience: Two plus years' experience social service delivery with elders preferred.
