Overview:

Comfort Keepers of Anoka and White Bear Lake is seeking a highly skilled and empathetic Client Care Coordinator to play a vital role in ensuring our clients receive exceptional care while expanding our reach within the community. This dynamic position requires a self-driven individual with a passion for senior care, skilled in relationship building, and driven to make a difference in the lives of others.

Key Responsibilities:

  • Perform in-home consultations and assessments to establish services for potential clients, implementing contracts and agreements to meet sales goals.
  • Participate in intake call handling with the sales team to discuss Comfort Keepers services and schedule in-home consultations.
  • Engage in community activities, including industry-related committees, boards, community projects, and fundraising, to represent Comfort Keepers and initiate and maintain referral relationships.
  • Perform new networking/sales calls to assess relationship possibilities with referral sources, targeting accounts such as Assisted Living Facilities, Nursing Homes, and Skilled Nursing Facilities.
  • Conduct ongoing care coordination activities, including reassessments, client advocacy, and spot checks.
  • Participate in community education presentations and work with team members to promote Comfort Keepers' expertise and brand awareness.
  • Assist in developing and maintaining a database for relationship management.
  • Participate in on-call rotation for sales, as assigned.

Requirements:

  • Minimum 2 years related experience and/or training.
  • Experience in senior care or other service industry highly preferred.
  • Experience in hospital or transitional care settings is preferred.
  • Must possess and demonstrate excellent communication skills and a positive, professional business image.
  • College degree in health care, marketing management, or a related field.
  • Requires proficiency in Microsoft Office applications and the ability to master additional software, including scheduling systems, calendars, and CRM software.
