$32-36K annually What Will You Do? The Family Support Partner’s primary purpose is to provide support, guidance, advocacy, and education to their assigned clients. Performance will be measured by your individual outcomes, achieving your individual targets/goals, your contribution to your overall program team/department and your buy-in to the culture of the organization. The Family Support Partner will do the following: Provide community resource expertise to the children and families, including identifying and collaborating with the community-based service providers and supports including use of NC Cares 360 or other tools as directed by the payer. Engage with the clinicians in the collaborative family assessment process (i.e., use of data from interviews, observations, interactions, and standardized measures to identify family strengths, needs and challenges). Promote family stabilization by working with the family to identify all needed and desired services ,

integrating service needs into the Child and Family Plan of Care, as appropriate, and addressing barriers to services as they arise. Enhance caregiver executive functioning skills (e.g. planning, organizing, managing time, focusing attention, regulating emotions, reflecting on progress) as needed and in consultation with the Clinician and Clinical Director. Maintain a reflective lens when engaging with the caregiver in order to understand his/her motivation, needs, and possible barriers to new services and supports. Requirements What qualifications do you need? The Family Support Partner must have the following: Bachelor’s degree in a human services field from an accredited university and one year of experience working with the target population; or Associate’s degree in a human service field from an accredited school and two years of experience working with children/adolescents/transition age youth; or High school diploma or GED and a minimum of four years of experience working with children/adolescents/transition age youth. Lived experience as a primary caregiver for a child who has/had mental health/ substance use disorder or intellectual/developmental disability is required. Experience in navigating any of the child and family-serving systems.
