In addition, this position is responsible for answering incoming phone calls, directing calls to the appropriate associate, handling the flow of correspondence, and the requisition of supplies as well as additional clerical duties. This position ensures the efficient day-to- day operations of nurse scheduling.The Administrative Clinical Assistant role is responsible for working collaboratively with the ANS Nursing team and pharmacy to provide seamless patient and referral source care through quality and coordination of care. The position is responsible for supporting the quality nursing team to assist with tracking and uploading of patient documentation and ensuring the information is received in a timely manner. The position is also responsible for periodic quality audits, data mining, patient record management and reporting.ESSENTIAL DUTIES AND RESPONSIBILITIES:Clinical Support Coordinator role specific: Works and coordinates with Intake, pharmacy and nursing personnel to ensure new referrals and ongoing nurse visits are scheduled in a timely, efficient manner. Creates initial nursing schedule and ongoing schedules as needed to ensure appropriate coverage for patients. Verifies patients are ready to schedule. Determines nursing dates available and confirms dates with patient. Updates patients on next visit or order status as needed. Coordinates with home health agencies to schedule patient visits. Answers telephones, prescreens, and directs the caller to the appropriate associate. Will transfer a caller to an associate's voice mailbox when the associate is unavailable. Takes and retrieves messages for various personnel. Creates and confirms Clinician Visit Log (CVL). Checks e-mails and faxes throughout the day for documents that need direction or attention. Provides callers with information such as company address, directions to the company location, company fax numbers, company website and other related information. Assists staff with administrative support when requested. Recognizes patients' rights and responsibilities and supports them in the performance of job duties; respects patient's rights to privacy and confidentiality. Completes assigned paperwork/documents as directed in a timely manner. Shares knowledge gained with other staff members and works as a team member. Interacts with others in a positive, respectful, and considerate manner. Performs other job related duties as assigned.Administrative Clinical Assistant role specific: Recognizes patients' rights and responsibilities and supports them in the performance of job duties, respects patient's rights to privacy and confidentiality. Reviews patient medical chart content for accuracy. Audits, tracks, monitors, and ensures completion of documentation. This is a document review process, not a clinical review. Works collaboratively with all members of the nursing team including the quality nurses, field nurses and directors to ensure patient care documentation, data collection, and coordination of care is completed. Ensures documentation is accurately maintained and processes are followed. Through the review, notifies the prescriber, nurse, and leadership of changes in the condition and/or needs of the patient and documents accordingly. Represents the company in the appointed office for any surveyor or audit team. Notifies leadership of any surveyor or auditor immediately. Ensures quality customer service. Shares knowledge gained with other staff members and works as a team member. Maintains patient records in compliance with HIPAA, state, URAC and ACHC regulations. Understands and adheres to established company policies and procedures. Participates in performance improvement activities for the enhancement of care delivery andoffers suggestions and assistance towards the continuous quality improvement of services offered to consumers.
