The Care Without Delay (CWD) Specialist is accountable for the overall KPWA care delivery of members who receive hospital services in partnership with the KFHP leader(s). The CWD Specialist sets high standards of quality performance for hospital care and actively ensures the standards are met. They organize the care of KP patients to maximize their health, improve quality of care all while reducing waste and excess costs. The CWD Specialist upholds a culture of service, quality, and efficiency. The CWD Specialist is expected to promote diversity and equity within the department as consistent with WPMG Board's commitment to equity and diversity. The CWD Specialist will collaborate with the Hospitalist Associate Medical Directors, Assistant Hospitalist Medical Directors, District Hospital Medical Directors, Care Management and our partner hospital administrations to support the CWD principles of timely, appropriate patient care. They will provide clinical CWD program oversight and they will address and escalate patient progression barriers. The CWD Specialist will partner with Care Management to facilitate the documentation of escalated issues for the purpose of tracking and trending. The CWD Specialist will support the Assistant Senior Medical Director of Hospital Based Services and health plan partner(s) in the development of appropriate reporting methodology and measures. The CWD Specialist will also facilitate closed loop communication to ensure that local teams are aware of the progress of escalated issues. The CWD Specialist will be expected to participate in local team meetings in the CWD Specialist capacity, bringing forward topics to further advance the CWD program and principals. The CWD Specialist will also attend program level meetings lead by the CWD Assistant Senior Medical Director of Hospital Based Services, their health plan partner(s), and the CWD program manager. The CWD Specialist will have a matrixed leadership structure with the Hospitalist Associate Medical Director and Assistant Senior Medical Director of Hospital Based Services. The CWD Specialist will have direct reporting relationship to the Hospitalist Associate Medical Director as in relates to inpatient operations and will be accountable to the Assistant Senior Medical Director of Hospital Based Services as it relates to the development, implementation, and ongoing monitoring of the CWD program. The CWD Specialist will function in a virtual capacity and may partner with multiple hospital locations to lead CWD huddles and rounds. The CWD Specialist job description may be revised over time based on operational needs and perform other duties as assigned. PRINCIPAL DUTIES & RESPONSIBILITIES

Responsibility Description Strategic Direction Promote organizational vision and strategic direction by ensuring CWD principles are upheld and communicated with providers and hospital staff. Ensure an integrated patient care collaboration model comprised of a set of processes and staffing practices that creates alignment and accountability across all providers and departments who care for patients in the hospitals. Develop and foster CWD strategies for rapid patient assessment, appropriate site and level of care, effective CWD rounding practices, safe and timely discharges. Ensure program adoption and sustainability with our physician team by providing local coaching, using common language, and addressing escalated concerns. Co-lead CWD rounds. Promote efficiency of practices using process improvement technology. Provide clinical leadership over data collection and reporting. Apply insights and trends from CWD rounds to guide improvement in the quality of care delivered. Physician Leadership Accountable for the overall program engagement. Provide real-time peer review feedback, recognizing excellence, promoting CWD skill development, and celebrating successes of the medical teams. Be available to mitigate escalations during normal business operating hours: Monday -Friday, 8am-5pm. Ensure CWD foundational elements of care delivery. Support CWD program consistency across the team. Support efforts to continually improve the delivery of services in KFHP-partner hospitals. Partner with the local Associate Medical Director to coach CWD skills in all staff. Model CWD practices and promote excellent patient centered care by removing patient care delays. Inspire and encourage all staff to adopt CWD skills and embody the CWD approach to patient care. Partnership Model and promote an effective CWD partnership between WPMG and KFHP Case Management. Partner with Care Management to achieve common CWD goals for service, clinical quality, and access. Collaborate with DHMD to support operational processes, clinical care, service or facility delay issues. Partner with the local Associate Medical Director to support CWD goals and team skill level. Operational Excellence and Quality Promote use of CWD practice guidelines and evidence-based medicine. Encourage efficiency of CWD medical practices using process improvement technology. Promote affordability for our patients by supporting CWD strategies. Partner with the DHMD, local Chief leadership and Care Management to ensure practices and procedures for effective care of patients to minimize inappropriate use of hospital-based services. Ensure optimal practice environment (CWD tools, processes, escalations) to promote highly efficient physician practices. Coach CWD practices and utilize CWD processes and tools. Identify to local Associate Medical Director compliance concerns with CWD program requirements. Medical Group Brand and Image Translate and communicate KP mission, the CWD vision and performance to staff. Celebrate successes, recognize excellence in performance, and communicate accomplishments to build pride. Partner with KP to improve the organization's reputation and image by promoting Care without Delay excellence and expertise of the medical group. Partner with the DHMD to ensure CWD goals are met with the intent to enhance our hospital relationship and improve patient care. JOB SPECIFICATIONS
