Position Overview – Registered Dietician: • Represent Empyrean Hospice with the utmost professionalism and compassion • Develop and implement a nutritional plan of care based on a comprehensive assessment of nutritional needs as part of the patient's total plan of care; provide evaluation and reporting appropriately • Work with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers; provide instruction on nutritional principles, dietary plans, food selection, and economics, and adapt plans to the individual's illness, as indicated • Communicate appropriate dietary history and nutritional care data including assessment, treatment plan, progress notes, and discharge summary in the clinical record • Ensure standards of ethical business and clinical practice are prioritized Who is Empyrean Hospice?
